Job Description A Community Engagement Coach provides one-on-one services to individuals with Intellectual/Developmental Disabilities.
The services are designed to increase a client's engagement community activities of their choosing.
The Coach coordinates information, develops connections, provides referral/outreach and facilitates inclusion of individuals with developmental disabilities.
Work Role and
Responsibilities:
Work with clients, families, DDA Case Managers and CE Coordinator to improve client access to community.
Work one-on-one with clients to achieve community access goals.
Effectively communicate with peers, clients, families, case managers and supervisor.
Maintain case notes.
Report monthly on each client's goals, success, hurdles, outcomes and next steps.
Qualifications and Education Requirements:
Experience in providing direct assistance to individuals with disabilities, or comparable training, education/skills Demonstrated skill working effectively and communicating clearly with individuals with intellectual/developmental disability and their families Motivated, organized and possess good time management abilities Proficiency in Microsoft Word Must pass DSHS background check Job Types:
